Levels of Care at a Glance

Below are guidelines which will help you see at a glance our levels of care. These are not all
inclusive and persons may need assistance with all or some of these areas in different degrees.
Actual Level of Care will be determined by a staff and families after an assessment by our team.

 
   


Level 1: Basic Physical and / or Dementia Care

Three delicious home cooked meals daily with snacks
Caring help with dressing
Caring help with bathing
Caring help with toileting
Sensitive medication administration
Sensitive daily reminders
Bi-weekly laundry
Daily Housekeeping
Supportive 24 hour supervision
Fully secured environment
Challenging and fun daily group activities
Challenging and fun daily individual activities
Specially trained social cueing
Special Memory Care understanding and reminiscence
Delicious special diets
Monthly workshops and / or get togethers with families

Level 2: Moderate Physical and / or Dementia Care
All of Level 1
Caring daily bathing, if necessary
Clean daily laundry
Sensitive assistance with transferring (walking)
Sensitive incontinence management (moderate)
Sensitive help with eating
Extensive appropriate social cueing
Understanding of Alzheimer’s Disease and other
dementias for those with moderate level dementia
Occasional two person transfers with care
Hospice care (as needed)

Level 3: Total Physical and / or Dementia
All of Level 1 and 2
Two person transfers with care
Sensitive wheel chair assistance
Understanding of Severe Alzheimer’s Disease
or Other Dementia
Sensitive incontinence management (total)
Sensitive help with eating
Daily one on one interactions
Fifteen minute checks
Wound care
Special needs
Range of motion daily
Re-positioning every two hours
